Assesing your needs

We will work with you to put together a service user plan. This plan will go through information about the support you require how often and the times this is required. We make this plan completely individual the child or young person requiring the support this will take into considerations, their cultural, religious and dietary needs. Under current Health and Safety legislation we are required to carry out an assessment of your home, to make sure that it is safe for your home care worker to work with your child. We will provide a copy of the plan for the home. This will contain all the relevant information for the care and support to be provided.

This will include:

  • Service user plan
  • Contact information
  • Emergency details card
  • Medication information
  • Risk assessment forms
  • Pocket money forms
  • Daily recording sheets
  • Incident and accident sheets
  • Details of opening hours and emergency on call numbers

What happens if my childs needs a change?

We recognise that people’s needs change and we will keep up to date of your child’s needs by: Regular supervisions with your support staff Reviews with your social worker Updating of the Risk Assessments Review of the service user plan Regular telephone communication
